KaosEngineer
Is the modchip actually working and enabled? Which modified Xbox BIOS/kernel version is installed? With error 16 and 21, they normally indicate that the stock MS dashboard is attempting to run; however, some of the files are missing and/or corrupt on the hard drive. KaosEngineer
5713 is a stock Xbox BIOS version number. For some reason, the modchip is not working. Do you have a picture of it? How is it installed: pin header, wires, solderless adapter?

You need to get the modchip working first before you do anything else. Hot swapping will not help to get the console running in a modded state. Smaley
This is the underside of the modchip
KaosEngineer
Where's the D0 wire from the modchip connect to the motherboard. The BT wire - where does it connect to the motherboard? As I believe it is currently configured, you have to hold the power button for longer than 1 second for the modchip to be enabled. Try powering on the console with a long power button press. What happens? The modchip can be configured so it is always enabled instead of requiring the long power button press to enable it.
bolofski
just typing the same kaos - check the connections from the modchip to the board, particularly the d0 wire, if you can solder i would also suggest you re-do the modchip with a link from bt to ground so that the chip is always on, as the way they were originally installed was enable / disable depending on whether you short tapped the power button or held it for a second, this pic shows the bt to ground link
