I've been having exactly the same problem for over a year now. I searched the internet for a solution to this issue fruitlessly but I think today I finally figured out where the problem lies, at least on my end. Maybe it's the same for you too. Turns out my TV just hates the 480p signal for some reason, when I'm in the Dashboard (Xbmc4Gamers) or in a game that runs in 720p it runs without issue. Hopefully this post if of some help to you.
